Policies

Your child has been referred to our clinic to be seen by a pediatrician. Please note that this referral does not mean that you are a primary patient for this physician. Our clinic has policies that are expected to be followed by all who attend/join our clinic.

  • The referral provides you a consultation with a pediatrician to review and assess your child only for the reason your child was referred.

  • You must follow up as instructed by the Pediatrician,. If you do not comply with their recommendations and you are not seen within 12 months, it will be assumed your child is doing well and your file will be closed. You will be required to obtain a new referral letter from your family doctor if no contact is made within 12 months from the last appointment before being able to schedule another appointment with the Pediatrician at Sunnyhill Pediatric Clinic.

  • Families are responsible for booking follow-up appointments . Sunnyhill will not call to remind families to book.

  • Once the Pediatrics physicians feels your child’s issues have been appropriately addressed, they may decide to direct you back to your family doctor for primary care.

  • Your child MUST be present for all appointments.

  • If you are 15 minutes or later for your appointment, you may be asked to reschedule.

Physicians and front-staff do NOT tolerate abusive behavior, in any form. You will be dismissed/fired from the clinic should you not respect the office staff or Pediatricians.

Fees/Invoices

There are many children waiting for appointments and the staff requires adequate time to fill cancelled appointment times. There are penalty fees for patients who do not provide enough notice for cancelling/rescheduling appointments.

Please see the list below:

Follow up appointments require a minimum of 24 hours notice. Without proper notice the fees are as follows:

  • 15 minute appointments - $50.00

  • 30 minute appointments - $100.00

All consults require a minimum of 48 hours notice. Without proper notice the fees are as follows:

  • 60 minute appointments - $300.00

  • 90 minute appointments - $350.00

We also charge a fee for prescription renewals over the phone. The fees associated to that are:

For standard prescriptions - $35.00

For triplicate prescriptions - $45.00

** If you are unable to pay the above prescription fees you will need to book an appointment to be able to receive the prescription**

All requests for copies of a patient chart for families and/ or another physician is subject to the chart copy fee. This cost includes administration time, chart review, cost of USB, and mailing. Please note that we only provide a chart copy via USB devices, which are password-protected and mailed out standard Canada Post. The fee for the chart copy is $35.00.

** For all payments to the office or physician, we accept Cash, Debit, or Credit**

***Please note our office charges a $100 fee to complete a Disability Tax Credit Form***

Running Late?

If you are running late (5-10 mins) please contact our office to inform us. Please note that if you are more than 15 mins late for your appointment you may be asked to reschedule your appointment.
